Consent to U.S. Tax Reporting via Electronic Format

For all current Mainsail investors, your 2012 Federal and State Schedule K-1s are now available. By clicking the “Agree” button below, you are also affirming your consent to receive your U.S. tax schedule K-1 via electronic format. Such Consent is applicable for the duration of the partnership, unless you specifically withdraw the Consent in writing by notice to the partnership’s CFO. Withdrawal of the Consent would be effective after receipt of confirmation from the partnership of your withdrawal of the Consent and its effective date, and would not apply to any tax reporting provided prior to the effective date of withdrawal. The partnership will also discontinue electronic reporting upon your withdrawal from the partnership. When you access your electronic format K-1, you can save the file in your records or print a paper copy, or you can send a written request for the partnership to provide you with a paper copy. You may receive your K-1 tax reporting via paper format from the partnership if you do not consent to receive it electronically. Your Schedule K-1 will be available on the SmartRoom website for a minimum of three years after issuance. Your Schedule K-1 may be required to be printed and attached to a U.S. Federal, State, or local income tax return.
